Required Annual Parent/Legal Guardian Forms

Parents/documented legal guardians are required to complete two annual forms to ensure schools have the necessary and most up to date information on file, and that essential permissions and notifications have been completed for each student.

 

1. Student Verification Form

The Student Verification Form is a document which displays the information the school currently has on file for a child in MyEducationBC, the student verification system used by school districts and the province. Parents/ documented legal guardians must carefully review the form every year and make sure that the school has the most up to date information.

If changes are made to any of the following information items, parents/legal guardians must provide supporting documents:
• Home address
• Parental responsibility
• Legal or medical alerts
• Citizenship
A list of acceptable documents is available at https://westvancouverschools.ca/admissions/required-documentation/ Staff may request additional documentation at their discretion.

If supporting documentation is not provided, changes will NOT be made in MyEducationBC, the official student information system used by school districts and the provincial government to maintain a student’s permanent record.

Emergency Contact Section

The Student Verification Form includes the Emergency Contact Information, which is critical in the event of a personal or larger scale emergency or disaster. The data provided on the verification form will be used to generate the emergency information forms schools require on file for each student.

Please note:
• Only contacts listed will be authorized to pick up your child in the event of an emergency
• All emergency contacts must be 19+ years and aware of this role
• We recommend including at least one trusted contact who lives nearby
• In the event of a broad emergency, the school may implement a controlled release of students to ensure their safety and well-being. If this becomes necessary:
• Students will only be released to parents/guardians or emergency contacts listed on the form. If necessary, students may be released to authorized medical personnel
• Accurate emergency contact information is essential for your child’s safety. Please ensure this section is accurate and up to date.

2. Complete the Online Policies & Procedures Form

The online Policies and Procedure form overviews the essential permissions and notifications the school needs for each child.  Failure to submit the form will limit your child’s participation in classroom and school activities.

The form covers:

  • Basic student information
  • Permissions for school activities and programs
  • Permissions for technology and digital recourse use
  • Release of limited personal information

 

The Student Verification Form and an online Policies and Procedure form must be completed for EACH child.
