PERMISSION FORMS. Each year, parents/guardians must complete various forms for their children for the upcoming school year. The District Personal Information Verification, Emergency, Policy and Procedure Forms must be completed online. The personal information is gathered through three tabs (Student Tab, Family Tab, Alerts Tab). In addition, these online forms also cover use of school internet, Google Apps for Education consent and media parental consent form. Due to privacy laws, THESE E-FORMS MUST BE COMPLETED EACH YEAR. Failure to complete this form may result in your child being unable to go on walking field trips with their class, use an electronic device at school, or participate fully in some is some school activities. Your child’s school will also provide paper-based forms that need to be completed and returned to school no later than Friday, September 18.
